Permission to Publish On the Internet Form

As students, faculty, and staff become involved with Internet publishing, it will be necessary to have permission from parents or guardians to allow for student's names (first name only with possibly first initial of last name), works, and pictures to be displayed. These permission forms should be sent home by faculty or staff before the students become involved in publishing information on the Internet.

No personal information will be published on the Internet regarding a child's full name, address, phone number or family information.

Permission to Publish on the Internet Form

As part of a class project, we would like to include work done by your child on our school Internet home page. This project may include one of the following:

  • A picture of a work (such as an art project) or writings (such as a poem or story) completed by your child with the first name and last name initial only.
  • A picture of your child while attending the Shelburne Community School (either in class or during an event). We will only use your child’s first name and last name initial.
  • A photograph or video images of your child engaged in a performance or simulation in which your child might be identified by their specific roles.

We have, and will continue, to practice steps that promote safe use of the Internet with all levels of learning at SCS. Please note that this information may be accessible to anyone having Internet access. We will not use a child's full name and will not give out any information about the student other than that pertaining to the story or project referenced. This request is in addition to previous permission forms related to photographs or video images of your child (children).

We will not use any image or reference to a name of an individual student unless a permission slip is signed. If you have any questions please contact _________________ at 985-3331. Please answer and sign the bottom of the form and return to (hand deliver or mail):
